Lumbar Disc Replacement
Overview:
Lumbar Artificial Disc Replacement is a procedure where mechanical devices are implanted through the abdomen to replace damaged discs, intending to preserve movement that mimics a healthy, natural disc. Common artificial discs include Maverick, CHARITE, and Prodisc-L.
Advantages:
This procedure aims to preserve motion at the disc space, purportedly placing less stress on additional vertebral joints compared to a fusion operation. It is believed to mitigate problems that often develop in adjacent disc spaces post-fusion surgery, enhancing the overall post-surgical experience for patients.
Suitability:
Artificial disc replacement is generally suited for younger patients experiencing primarily discogenic pain. It is not recommended for:
- Elderly patients
- Osteoporotic patients
- Patients with facet joint disease
- Patients experiencing sciatica
- Patients with severely degenerate or collapsed discs
- Patients over 50 years of age
Post-Operative Care and Recovery:
- Post-Surgery: Patients may experience immediate pain relief, but muscle strength recovery might be gradual. Loss of sensation usually recovers quickly if the pressure hasn’t been severe.
- Activity: Mobility is encouraged the day after surgery. Patients should avoid sitting for prolonged periods and refrain from heavy exercise and lifting for the initial six weeks. Walking is recommended two weeks post-surgery.
- Wound Care: The wound should be covered for the first five days, avoiding prolonged soaking in water for the initial two weeks. Mild lower back pain around the wound and mild sciatic pain due to nerve inflammation are common but usually improve with time and may be alleviated with anti-inflammatory drugs.
- Review and Normal Activity Resumption: A review is typically scheduled six weeks post-operation, allowing most patients to resume their normal exercise programs.
Conclusion:
Lumbar Artificial Disc Replacement is a significant advancement for qualifying patients, offering a motion-preserving alternative to fusion surgeries. While suitable for a specific patient group, it holds the promise of minimizing postoperative complications and enhancing the quality of life by retaining natural disc movement. Understanding the postoperative care and adhering to recovery guidelines are crucial for optimizing the benefits of this innovative procedure.
