Thoracic Spine Surgery

Thoracic disc prolapse is relatively uncommon compared to disc prolapse in the cervical or lumbar spine, occurring with a frequency of approximately one in a million people annually. These discs tend to be chronic and calcified, posing a risk for acute spinal cord compression, leading to symptoms such as weakness, numbness, and tingling in one or both legs. When spinal cord compression is the cause of these symptoms, surgery is typically indispensable.

Types of Thoracic Spine Surgery:

Thoracotomy:

  • Approach: This method involves opening the chest, allowing for a direct view of the spine.
  • Procedure: The lung is collapsed to access the spine, the affected disc is removed, and the spinal cord is decompressed.
  • Stabilization: Typically, metal plates and screws are inserted to stabilize the spine post-operation.

Posterior Laminectomy:

  • Approach: This technique employs a rear approach to access the thoracic spine.
  • Procedure: The operation involves exposing the edge of the spinal cord and removing disc fragments.
  • Stabilization: Metal hardware may be inserted to perform a posterior thoracic fusion for spinal stabilization if necessary.

Objectives and Considerations:

The prime objective of thoracic spine surgery is to preclude any damage to the spinal cord, mainly when other alternatives are not available. These complex operations aim to relieve the pressure off the spinal cord by removing the affected disc and stabilizing the spine to prevent any future complications or instability. It is crucial for the patient to understand the nature of the procedure, its objectives, and the postoperative care required, enabling an informed and consensual decision for the surgery.
